3. A composite letter of Support to be prepared by the dean of the institution, a peer professional and a student or alumni representative. The letter should detail the applicant’s contribution to the scholarship of teaching and learning and describe the reasons the nominee should receive the award. This composite letter must identify the nominee’s past recognition and summarize the most recent course evaluations.
4. A suggested citation, thirty-five words or less, summarizing and highlighting the nominee’s achievement. A listing of career highlights – only list awards or accolades not workshop / CDE attended.

The Guident Award is given for distinguished achievement, leadership, research and patient care. The achievements must be identifiable as lasting, impactful, and substantial. Only living nominees will be considered.

Nominees are to be identified and recognized initially within individual institutions. Ideally, identification, review and recognition of the candidates will be coordinated by a team comprised of: Senior Faculties, The dean of the institution, and a peer teaching professional.
