  1. A letter of justification – To be written by the Principal, A statement outlining the Vision and Mission of the college. The institutes’ educational and Infrastructural growth and development, and the teaching and learning philosophy.
  2. Brief Profile should include the following

  3. A list of Nominations submitted in each category from the College:
  4. A list of articles contributed to Guident magazine from the College.
  5. Letters of support : from the Alumni’s of college (1 UG and 1 PG) stating the year of graduation and enumerating how the institute has helped them in gaining knowledge and enhance their clinical skills during their course of study. Also describing the reasons why they consider their institute the best in the country. (Put Scanned Copies (Min 300 dpi) on 1 MS word file)
  • For Senior Academician: Min 8 or more years experience as academician required
  • For Junior Academician: Min 3 & Max 8 Years of experience as academician required

The Guident Award is given for distinguished achievement, leadership, research and patient care. The achievements must be identifiable as lasting, impactful, and substantial. Only living nominees will be considered. Nominees are to be identified and recognized initially within individual institutions. Ideally, identification, review and recognition of the candidates will be coordinated by a team comprised of: Senior Faculties, The dean of the institution, and a peer teaching professional.   1. A bibliography of significant publications. (Please omit any secondary publications and non-indexed journals from the nominee’s bibliography and do not include complete articles as part of the nomination.)
  2. At least three letters of support from individuals familiar with the work. (Put Scanned Copies (Min 300 dpi) on 1 MS word file)
  3. A composite letter of Support to be prepared by the dean of the institution, a peer professional and a student or alumni representative. The letter should detail the applicant’s contribution to the scholarship of teaching and learning and describe the reasons the nominee should receive the award. This composite letter must identify the nominee’s past recognition and summarize the most recent course evaluations.
  4. A suggested citation, thirty-five words or less, summarizing and highlighting the nominee’s achievement. A listing of career highlights – only list awards or accolades not workshop / CDE attended.
  5. ENCLOSE AN ORIGINAL ARTICLE / OR CASE REPORT FOR PUBLICATION IN THE GUIDENT JOURNAL (AUTHOR GUIDELINES), IT SHOULD NOT HAVE BEEN PUBLISHED IN ANY OTHER JOURNAL. (MANDATORY- FOR ACCEPTANCE OF NOMINATION)

  2. Brief Profile of the Head of the Department.
  3. A suggested citation, thirty-five words or less, summarizing and highlighting the Department’s achievement. (Mandatory)
    • A bibliography of significant publications from the Department. (Please omit any non-indexed journals from the nominee’s bibliography and do not include complete articles as part of the nomination.) Highlight the list of articles contributed to Guident Journal by any faculty or PG student of the department.
  4. A composite letter of Support to be prepared by the dean of the institution, and a PG student or alumni representative. (Put Scan copies (Min 300 dpi) on 1 MS word file)
  5. ENCLOSE AN ORIGINAL ARTICLE / OR CASE REPORT FOR PUBLICATION IN THE GUIDENT JOURNAL(AUTHOR GUIDELINES), IT SHOULD NOT HAVE BEEN PUBLISHED IN ANY OTHER JOURNAL. (MANDATORY- FOR ACCEPTANCE OF NOMINATION)

    • A composite letter of Support to be prepared by the dean of the institution or the Head of the Department and a CO- PG representative. The letter should detail the applicant’s contribution to patient care in the department, excellence in learning and describe the reasons the nominee should receive the award. This composite letter must identify the nominee’s past recognition and summarize the most recent course evaluations.(Scanned Jpg files (Min 300 dpi) put on 1 MS word file)
    • Research Project and funding: Write the research topic and briefly outline the progress made and the funding agency.
  4. A bibliography of publications. (Please omit any secondary publications and non-indexed journals from the nominee’s bibliography and do not include complete articles as part of the nomination.)

    • A composite letter of Support to be prepared by the dean of the institution or the Head of the Department and a CO- PG representative. The letter should detail the applicant’s contribution to patient care in the department, excellence in learning and describe the reasons the nominee should receive the award. This composite letter must identify the nominee’s past recognition and summarize the most recent course evaluations.
    • Certificates of any Workshop or Course attended during Internship

  4. Enclose an original article / or case report for publication in the Guident journal (Author Guidelines), it should not have been published in any other journal. (MANDATORY- for acceptance of nomination)
